Due to the constant changing of insurance plans and benefits, our office will verify eligibility and ask you to contact your insurance carrier for benefits.

While we accept most insurance plans, we are participating providers for Delta Dental and Medicaid. If the procedure will be covered by your medical plan, please contact your medical insurance carrier to obtain the requirements in order for your procedure to be covered at our facility.
